In the United States, hospitals annually report over 5 million cases of infectious-disease-related illnesses: clinical microbiology laboratories in these hospitals are engaged in detecting and identifying the pathogenic microorganisms in clinical specimens collected from these patients with suspected infections. Clearly, the timely and accurate detection/identification of these microbial pathogens is critical for patient treatment decisions and outcomes for millions of patients each year. Despite an appreciation that the outcome of an infectious-disease-related illness is directly related to the time required to detect and identify a microbial pathogen, clinical microbiology laboratories in the United States as well as worldwide have long been hampered by traditional culture-based assays, which may require prolonged incubation time for slowly growing microorganisms such as Mycobacterium tuberculosis. Moreover, traditional culture-based assays often require multiple steps with additional time needed for discernment of species and/or detection of antimicrobial resistance. Finally, these traditional, slow multistep culture-based assays are labor-intensive and required skilled clinical microbiologists at the bench. Over the past several decades, advanced molecular techniques in diagnostic microbiology quietly have been revolutionizing the practice of clinical microbiology in the hospital setting. Bioactive materials, or biomaterials, have the ability to interact biologically with the tissue to which it is inserted, and to stimulate the deposition of mineralized tissue. The calcium phosphate-based ceramics were the first known materials in dentistry to have bioactivity, and currently, these materials are the most used for biomedical purposes, with different morphological characteristics. In dentistry, these materials have achieved immense importance by stimulating the deposition of osseous tissue in injured bone, and by having the ability to remineralize hard tooth tissues (enamel and dentin). Furthermore, repair materials based on aggregated trioxides mineral or on calcium hydroxide are classic biomaterials and widely used in dentistry, mainly in contact with the pulp tissue or periodontal ligament, for repair processes. However, various formulations of these materials appear all the time, in search of the ideal material. In general, bioactive materials have been shown to promote the release of calcium, sodium, silicon and phosphate ions, which are metabolized by the body, having effects such as angiogenesis and antimicrobial action, which can be improved depending on the composition of the material. Pulp tissue is a highly specialized dental tissue and is the subject of intense studies about the response to biomaterials. It is also understood that some systemic alterations in individuals have an influence on the action of bioactive materials during tissue repair processes. Thus, this book will address the use of different bioactive materials in dentistry, considering the performance of these biomaterials in the hard tissues of the tooth, and the response of the dental pulp, as well as the influence of the composition of these materials and of the individual's systemic alterations in bioactivity and in antimicrobial activity. The book at hand is an international text on advanced oral and maxillofacial implantology. It is the fruition of over 40 specialists from 8 countries. The procedures presented herein are meant for expert implantologists; these procedures (including sinus floor grafting and ridge augmentation, etc.) are not for beginners or the novice; and require a great deal of skill, training and experience to perform. Although the majority of these operations require only local anesthesia and sometimes sedation in an outpatient setting, a significant number of these procedures are major operations and need to be performed by trained OMF surgeons and may require hospitalization and treatment under general anesthesia. Recruiting national and international world-renowned experts in implantology from 8 countries to write the chapters of this book was indeed a difficult undertaking. The authors generously present their cases and state-of-the-art techniques in an educational manner; actual cases were treated and their outcomes are presented thus allowing the surgeon to choose the technique which best addresses the dilemma at hand and yields the best result for that particular case. Problems commonly arise when there is insufficient bone to place implants.
