School Health Statistics - Pennsylvania Department of Health -- Chronic conditions data is at the county level, however Staffing: Certified School Nurse (2012/13 to 2016/17) and Supplemental Staff (2012/13-present) at the school level.

Philadelphia Vital Statistics Report - City of Philadelphia, Public Health  --  2000-present. Births and deaths by census tract and ZIP code.
 

Community Health Data Base  - ZIP Code tabulations from SEPA HHS microdata. The Southeastern Pennsylvania Household Health Survey, the main part of the CHDB, collects information on health status, personal health behaviors, and access to and utilization of area health services from approximately 10,000 households throughout Philadelphia and Bucks, Chester, Delaware, and Montgomery Counties. The oldest ongoing metropolitan health survey program in the United States, the SEPA HHS program started in 1983 as a quadrennial program, changing to a biennial survey in 1994 with the most recent cycle conducted in 2015. - PennKey required.
